It depends on where the drain sits relative to the sewer line. If it is below, an ejector pump handles it — that is normal and it is priced openly. We establish which situation your house is in at the walkthrough, before any number exists.
If any room down there is going to be a bedroom, yes — it is code, and it is what a home inspector looks for when you sell. We would rather quote it than leave it out to look cheaper than the next contractor.
